designing a questionnaire

The data we collected was from a sheet that we had been given and then handed around the classroom and we filled in the sheet with our answers.  We had to tick boxes or simple sentences  and  both open and closed questions. but it could be changed, by the amount everyone could have wrote and this would be a closed question. and some questions didn't give enough options and some peoples answers were not on the sheet and this also made the questions longer.
